What is Immunotherapy?

Immunotherapy is a type of cancer treatment that helps your body’s immune system recognize and attack cancer cells.

Your immune system is designed to find and destroy abnormal cells. But cancer can develop ways to hide from immune cells or weaken the body’s natural defenses. Immunotherapy helps restore or strengthen those defenses so your immune system can better recognize and fight cancer.

Unlike chemotherapy, which acts directly on cancer cells, immunotherapy works by helping your immune system do what it was designed to do. Immunotherapy may be given alone or in combination with surgery, chemotherapy, radiation, targeted therapies, or other treatments.

Your immune system is your body’s natural defense against infections and abnormal cells, including cancer. Most of the time, it can recognize and eliminate damaged or abnormal cells before they become a problem. But cancer can develop ways to hide from immune cells or switch off the immune response. Immune cells are specialized white blood cells that work together to recognize threats, communicate with one another, and eliminate harmful or abnormal cells, including cancer. Each type has a unique role in protecting your body.

Your care team will consider your cancer type and stage, biomarkers or genetic changes, prior treatments, overall health, and immune system function before recommending immunotherapy.

How is immunotherapy given?

Many immunotherapies are given through an IV in a doctor’s office, outpatient clinic, hospital, or cancer center. Some are given as injections, pills, skin creams, or treatments placed directly into the bladder (intravesical).

At what stages of cancer can immunotherapy be used?

Immunotherapy may be used at many stages of cancer, depending on the type and treatment approval. It’s often used for advanced or metastatic cancer, but some immunotherapies are also used before surgery (neoadjuvant therapy), after therapy (adjuvant therapy), or with other treatments to help reduce the risk of cancer returning.

What are common side effects of immunotherapy?

How do doctors know if immunotherapy is working?

Your health care team will monitor your progress with checkups, lab tests, and imaging scans. Sometimes tumors may look larger before shrinking, a temporary effect called pseudoprogression. Your doctor can help interpret changes over time.

Does immunotherapy cure cancer?

Immunotherapy can be highly effective for some people, but it does not work for everyone. For some, it may lead to long-term remission. For others, it may help manage cancer, extend life, or improve quality of life.

Can people with autoimmune diseases or organ transplants receive immunotherapy?

Sometimes, but these situations require special care. People with mild or well-controlled autoimmune conditions may be able to receive immunotherapy with close monitoring. For organ transplant recipients, immunotherapy can increase the risk of transplant rejection, so it is considered only in select cases with specialist guidance.

How much does immunotherapy cost, and will insurance cover it?

Costs vary widely based on the treatment, schedule, insurance coverage, and care setting. Immunotherapy can be expensive, but many insurance plans cover approved treatments.
